An estimated five to eight million couples in the United States have difficulty conceiving. The sexual revolution of the sixties and seventies produced an epidemic of sexually transmitted diseases that caused fallopian tube dysfunction. Then, throughout the 1980s and 1990s, more and more couples delayed having children, making the biological clock a key factor in preventing pregnancy. My infertility patients today are an average of ten years older than when I first opened my practice in the seventies. These, along with other factors, create challenges for the modern reproductive endocrinologist. And we continue to struggle to meet them. As a reproductive endocrinologist who has worked with infertile couples for the past thirty years, I have had the privilege of participating in the growth of this specialty. I have witnessed a series of amazing technological breakthroughs that have made possible things that less than a generation ago were considered science fiction. For example, the development in the sixties of techniques for measuring minute quantities of hormones circulating in the bloodstream.
